Car Accident (MVA) Massage Therapy
In the state of Oregon, your car insurance is legally required to pay for all medical costs associated with your car accident (MVA) — including, in many cases, massage therapy.
Main Squeeze Body Lab requires an assessment and massage therapy prescription from a medical doctor, nurse practitioner, chiropractor, or physical therapist before providing services with no out-of-pocket costs. Please have your prescribing provider email their recommendation to anna@mainsqueezebodylab.com, or send a copy or photo.
Alternatively, you may pay the Time of Service rate of $150 per hour, with reimbursement when or if billed claims are paid. Please note that reimbursement is not guaranteed and may take up to six months. For liability purposes, massage therapy will not be provided within two weeks of a motor vehicle accident without an assessment.
To ensure your full treatment time, please complete your intake form prior to arrival. Existing clients must complete a new form specific to the incident. For successful billing, ensure your home address, claim number, and claim contact information are accurately listed. Your intake form and treatment notes will be shared with the insurance company billed for the service. Additional health information unrelated to the accident may be shared verbally if you prefer it remain private.
Please note that the cancellation and late policy applies. Cancellations or reschedules made within 48 hours of the appointment will incur a $25 fee. Same-day cancellations and no-shows will be charged the full Time of Service rate of $150. Insurance cannot be billed for missed services. Exceptions may be made in cases of emergency or illness at the provider’s discretion. Please arrive on time to receive your full 60 minutes of table time, as late arrival may result in a shortened session.
Main Squeeze Body Lab does not bill medical insurance. The above applies only to clients involved in an auto accident with an active auto insurance claim. HSA and HRA are accepted.
